Allocation Model – Request

Current model information and selection

• Information advising what model is currently assigned to the account
• Next rebalance date
• Link to cancel future rebalances to the current model
• Table will display showing the fund number, fund name, percentages allocated for future contributions and the model allocation percentages that currently apply to the account is displayed
• The drop-down menu displays the list of available models.  To select a different model, click the drop-down box and click on the selected model.  The table will refresh to display the funds that are contained in that model.  Once you have selected the new model, click “Continue” to go to the “Funds Acknowledgement” page
